- Submit the survey and entry form to receive one entry into the Monthly Draw Period that applies to the date you submitted your survey.
- Ensure that you complete the survey within seven days of your purchase, since the receipt code will expire after that time.
B) Phone Entry
- Call 1-800-531-2928 to complete the survey through the automated telephone system.

Who Is Not Eligible?

- Immediate family members of any of these individuals are also not eligible.
- Immediate family includes a husband, wife, spouse, mother, father, brother, sister, son, or daughter, whether or not they live in the same household, since this rule helps maintain fairness and avoids any potential conflict of interest.
- Anyone who lives in the same household as an ineligible employee or family member is also not able to enter, as living in the same residence could allow access to information that could influence the contest.
Verification of Eligibility?
- The Sponsor may request proof of identity and eligibility at any time during the contest, and if an entrant does not provide the required documentation when asked, the Sponsor may disqualify that person at its own discretion in order to maintain the integrity of the contest.
How Potential Winners Are Selected in Storeopinion.ca Survey No Frills?
- The Contest Administrator will hold a random draw on each Monthly Draw Date listed in the chart provided in the rules.
- The draw will take place at about ten o clock in the morning Eastern Time at the Contest Administrator office in Toronto, Ontario.
- During each draw, two eligible entrants will be selected from all entries that were received during that specific Monthly Draw Period.
- Entries are only valid for the month in which they were submitted. Any entry that did not win in a previous month will not move forward to the next month draw.
What Happens After a Name Is Selected?
- Before being declared a potential winner, the selected entrant must correctly answer a time limited mathematical skill testing question without help from anyone or from any device.
- The selected entrant must also sign a declaration and release form that confirms they followed all contest rules and that they accept the prize as awarded.
- Each potential winner will receive a phone call within four business days after the Monthly Draw Date.
Reasons a Potential Winner May Lose the Prize?
A potential winner may be disqualified if any of the following happens.
- They do not answer the skill testing question correctly.
- They do not complete and return the declaration and release form within the required time.
- They refuse or are unable to accept the prize.
- They are found to have broken any contest rules.
- The Sponsor or Contest Administrator cannot reach them because phone email or other communication is not answered or is returned as undeliverable.
If a potential winner is disqualified, the Sponsor may hold another random draw from the remaining eligible entries in that same Monthly Draw Period. The same rules will apply to the new selected entrant.
If a prize is not claimed by January thirty one two thousand twenty six, it will not be awarded.
